How to Access Your 1095C For Tax Season

Benefit-eligible employees are now able to access their Form 1095-C in Workday for the 2020 tax year. Form 1095-C is the Affordable Care Act-Employer Provided Health Insurance Form, which provides verification that LG Health offered employees the minimum required health insurance coverage, during the 2020 calendar year. Form 1095C is not required to complete your tax filings, but could be requested by the IRS at a later date.

Follow the steps below to view and print your Form 1095-C:
1. Log into your Workday account
2. In the top right hand corner click on your photo
3. Select View Profile just below your name
4. On the left hand side, under your photo and name, click on Actions
5. Hover over the Benefits menu option
6. Scroll down to the last option, View My ACA Forms
7. Click the View/Print button on the right hand side of the page
8. Click on the Notify Me Later button in the middle of the screen
9. The link to the PDF version of your Form 1095-C will appear in the upper right hand corner, click on the PDF link, open and print.

Your 1095C can also be found on the Benefits Worklet from your home page in Workday.
1. Log into your Workday account
2. Click on the Benefits Worklet icon
3. Under the View section, click on My ACA forms
4. Click the View/Print button on the right hand side of the page
5. Click on the Notify Me Later button in the middle of the screen
6. The link to the PDF version of your Form 1095-C will appear in the upper right hand corner, click on the PDF link, open and print.

Employees who did not select a receipt preference for this form of “Electronic Only” will be receiving this Form 1095-C at their home address. Employees may also download this form through Workday for immediate receipt – please follow the directions above.
