LG Health Benefit Extras*** is a website that highlights and manages voluntary benefits available to LG Health employees.
Sign up for LG Health Benefit Extras to ensure that you receive the most up to date voluntary benefits , offers, and discounts available. Employees* will be able to use their LG Health log-in credentials to enroll in LG Health Benefit Extras.

Identity Theft Protection
Voluntary Disability Insurance program is designed to complement your LG Health Short-Term Disability (STD) benefit accrual and is paid through post-tax payroll deductions. Employees must work a minimum of 20 assigned hours per week to apply and premiums are based on an employee’s age at the time of enrollment, and the length of the benefits period chosen. This program covers illnesses and off-the-job injuries after exhausting accrued Paid Time Bank and Short-Term Disability balances.

Paid Time Off
Based on you and your family’s income replacement needs, there may be a need for Supplemental Term Life Insurance protection.
You become eligible to purchase supplemental life insurance through Securian (formerly Minnesota Life) on your date of hire. You may purchase supplemental term life insurance in multiples of your salary – the lesser of 5x your base salary or $1,500,000 – in addition to the life insurance coverage that Lancaster General Health provides. You must complete a Statement of Health for amounts greater than $400,000 or 2x your basic annual earnings(whichever is less). Note: If you apply for coverage after your first 30 days of employment with LG Health, you must complete a Statement of Health, for any coverage amount.
You must use post-tax dollars to purchase Supplemental Term Life Insurance.
For added convenience, use payroll deduction for your Securian (formerly Minnesota Life) premium payments.
You may also purchase life insurance protection for your spouse and children. Post-tax dollars are used to purchase this additional protection.
For your spouse: You may buy up to a maximum of $250,000 as long as the amount does not exceed 50% of your supplemental life insurance amount, after completing a statement of health $30,000 is available to purchase for a spouse, as guaranteed issue.
For your children: You may buy $5,000 or $10,000 of coverage for your dependent children between the ages of 14 days and 26 years.
