The Family and Medical Leave Act (FMLA) – Eligibility: 12 months of service, working 1,250+ hours

The Family and Medical Leave Act (FMLA) entitles eligible employees to take unpaid, job-protected leave for specified family and medical reasons with continuation of group health insurance coverage under the same terms and conditions as if the employee had not taken leave. Eligible employees are entitled to twelve workweeks of leave in a 12-month period for:

  • the birth of a child and to care for the newborn child within one year of birth;
  • the placement with the employee of a child for adoption or foster care and to care for the newly placed child within one year of placement;
  • to care for the employee’s spouse, child, or parent who has a serious health condition;
  • a serious health condition that makes the employee unable to perform the essential functions of his or her job
  • any qualifying exigency arising out of the fact that the employee’s spouse, son, daughter, or parent is a covered military member on “covered active duty”

Continuation of Benefits while on Leave of Absence (up to and including 12 months of coverage)

Continuation of medical, dental and/or vision coverage begins on the first day of absence due to illness or injury. If an employee ceases to make timely payments of required contributions, cancellation of coverage may result. Employees are provided a grace period of 90-120 days to make payment, in the event of financial hardship.

Continuation of Benefits while on STD

Any pay periods unpaid resulting in no pay for benefit premiums will be calculated in ADP. Upon your return to work, your missed benefit premiums will be recollected through payroll. Upon entering LTD you will be billed directly to pay premiums to WageWorks. See Continuation of Benefits while on LTD below.

Continuation of Benefits while on LTD

Upon the first No Pay pay period, WageWorks will be notified to issue direct billing notices to your home address as listed in Pennfor. These direct billing notices are to pay your medical, dental, and/or vision benefits while not receiving a paycheck. Cancellation of coverage may result the date the employee ceases to make any required contributions or payment arrangements.
