UPHS Benefit Extras program – Eligibility: All employees
UPHS Benefit Extras*** is a website that highlights and manages voluntary benefits available employees.
Sign up for UPHS Benefit Extras to ensure that you receive the most up to date voluntary benefits, offers, and discounts available. Employees will be able to use their LG Health log-in credentials to enroll in UPHS Benefit Extras program.
The UPHS Benefit Extras program includes:

- Pet Care: Some loved ones have fur! Pet Insurance can help you save up to 90% on the cost of vet bills.

Voluntary Short-Term Disability Insurance – Eligibility: 0.5 FTE and first of the month following date of hire
Voluntary Disability Insurance program is designed to complement your LG Health Short-Term Disability (STD) benefit accrual and is paid through post-tax payroll deductions. Employees must work a minimum of 20 assigned hours per week to apply and premiums are based on an employee’s age at the time of enrollment, and the length of the benefits period chosen. This program covers illnesses and off-the-job injuries after exhausting accrued Paid Time Bank and Short-Term Disability balances. Information is part of benefit elections in ADP.

Voluntary Long Term Disability – Eligibility: 0.8 FTE and $75,000+ Base Salary and during Enrollment period
Based on you and your family’s income replacement needs, there may be a need for Supplemental Long Term Disability.
Employees (who meet certain salary requirements) have the option to purchase supplemental Long Term Disability coverage through UNUM that may increase the LTD replacement income to 75% of base monthly salary and bonus compensation. Voluntary Life Insurance – Eligibility: 0.5 FTE and 30 days of service
Supplemental Term Life Insurance
Basic Life Insurance is automatically provided to employees at 1x their annual base salary. You have the opportunity to elect additional coverages detailed below. Keep in mind, you cannot be covered as both an employee and a dependent. If you are eligible for UPHS Employee Life coverage, you do not qualify as an eligible dependent for Spouse Life coverage.
Supplemental Employee Life coverage in $10,000 increments up to the lesser of 8x your annual base salary or $1 million. For increases more than $20,000 or benefit amounts greater than $250,000, or 2 times your base annual earnings, whichever is less, requires Evidence of Insurability (EOI).
You must use post-tax dollars to purchase Supplemental Term Life Insurance.
For added convenience, use payroll deduction for your Securian (formerly Minnesota Life) premium payments.
Supplemental Spouse & Dependent Child Life Insurance*
You may also purchase life insurance protection for your spouse and children. Post-tax dollars are used to purchase this additional protection.
Spouse Life coverage equal to $10,000, $25,000, $50,000, $100,000, $175,000, or $250,000. Coverage of more than $50,000 requires Evidence of Insurability.
Child Life coverage equal to $5,000, $10,000, or $15,000.
*Only available to employees who purchase supplemental term life insurance.
